There are 14 endings. Ending 15 unlocks after seeing every other ending/death in the game.
Puzzle Setup
Lighter
Go to Floor 6.
Take the burnt cigarette from the cigarette tray.
Go to the west wall of the elevator.
Click the No Smoking sign.
Can I have a lighter
Scissors
Go to Floor 8.
Talk to the doppelgangers and die during the first interaction.
Restart.
Return to Floor 8.
Click the doppelganger co-worker first.
Push them further
Floor 4 Bedroom
Exit the point-and-click screen using the door icon.
In the 3D map, turn around using the left or right arrow keys.
Find the door marked 4.
Press Z.
Sleeping Pill
Enter the bedroom on Floor 4.
Search the room for the sleeping pill.
Its location is randomized.
Check drawers, under the bed, and the bed sheets.
Find it before making noise more than three times.
Cake
Go to Floor 6.
Open the drawers.
Find the Job Application Questionnaire.
Enter:

Receive the piece of cake.
Drugged Cake
Combine the sleeping pill with the piece of cake.
Screwdriver
Get the scissors.
Go to Floor 7.
Cut open the right animal carcass.
Rat
Get the screwdriver.
Unscrew the vent on the east wall.
Take the rat.
CD
Get the lighter.
Get the drugged cake.
Go to Floor 3.
Use the lighter.
Click the middle of the mouth.
Throw in the drugged cake.
After the floor falls asleep, click the shining part of the teeth near the bottom right.
Take the CD.
Computer Passcode
Get the rat.
Go to Floor 2.
Put the rat into the hole.
The password is:RAISE
Floor 9 Access
Get the scissors.
Cut open the sewn part of the wall.
Insert the CD into the computer.

Click the floor.
Step into the dark.
Ending #4 – EATEN
Go to Floor 3.
Use the lighter.
Click the middle of the mouth or the shining teeth.
Feed it something other than cake.
or
Try to take the shining object from the teeth without putting the floor to sleep.
Ending #5 – PARENTS
Go to Floor 4.
Enter the bedroom.
Interact with objects until you make noise more than three times.
Avoid finding the sleeping pill before triggering the ending.
Ending #6 – RADIATION
Go to Floor 5.
Enter the pool floor.
Wait until the timer runs out.
Ending #7 – REJECTED
Go to Floor 6.
